Housing Navigator FSS (Temporary)

JOB TITLE: Housing Navigator Family Support Services (FSS) (Temporary)

REPORTS TO: Director of FSS & Disaster

JOB OBJECTIVE: This position has been developed as a response to housing instability in OKC that was caused by the COVID-19 pandemic. This Housing Navigator will provide services for clients who are not currently accessing traditional referral streams. This Housing Navigator will work to build collaborative relationships with community partners and help connect clients to housing and provide navigation to ensure that clients have required documents and support services, including employment and income streams. This position is grant funded for one year. Beyond a year, the position will be contingent upon ongoing housing instability related to the COVID-19 Pandemic and available funding.

This is a Temporary Grant Funded position expected to last until September 30, 2023. The position is currently funded until September 30, 2022.

The essential job duties include but are not limited to:

  • Specializes in prevention and housing services with clients who are experiencing housing instability or homelessness because of the COVID-19 pandemic.
  • Engages clients in the case management process that includes:
  • Building a relationship of trust and support
  • Completing screening, intake and assessment to identify barriers and build on strengths
  • Develop an individual service plan in partnership with the client that includes goals related to HUD grant compliance as well as focusing on long term self-sufficiency
  • Monitor and update the service plan as changes occur and track outcomes
  • Provide appropriate referrals to community resources and advocate on behalf of the client
  • Complete discharge planning process as appropriate
    • Maintains precise and accurate documentation of case management services, including client files and entries into the program database (Caseworthy) for every face to face or telephone contact or collateral contact by the next business day. This includes capturing data regarding causes of homelessness related to the Pandemic.
    • Familiar with eviction court processes and procedures as well as legal resources available including Legal Aid and OCU and makes appropriate referrals.
    • Advocates for client(s) during Coordinated Case Management (CCM) meetings through the Oklahoma City Continuum of Care (CoC) as appropriate.
    • Assists clients with completing lease agreements and obtaining other supportive documents such as identification, social security cards and so forth as required by leasing agents/property.
    • Assists clients in accessing and maintaining entitlements and benefits.

    Ancillary job duties:

    • Participates in outreach activities related to housing instability
    • Is familiar with community housing resources and is able to quickly assess and make appropriate referrals.
    • Establishes and maintains close collaborative working relationship with community resources; attends and participates in appropriate coalition and other community resource meetings.
    • Facilitates and coordinates supportive activities within Catholic Charities and community partners, including employment assistance programs, job readiness training, financial education, and behavioral health services, to meet the person’s needs, preferences, and goals to build upon his/her strengths.
    • Transports clients as needed.
    • Performs other related duties as required.

    SUPERVISORY RESPONSIBILITIES:

    There are no supervisory responsibilities with this position.

    MINIMUM QUALIFICATIONS FOR CONSIDERATION:

    • Undergraduate degree in social work or behavioral science preferred
    • Two plus years of previous experience in adult case management preferred.
    • Computer literate; experienced with Microsoft Office (Word, Excel), Outlook, Internet.
    • Fluency in Spanish and English preferred.
    • A combination of education and relevant experience may also be considered

    • · Ability to excel in a diverse, collaborative team environment and effectively communicate with staff, clients, donors, vendors, community agencies, etc. in a polite, courteous and cooperative manner, yet appropriate assertive as situation may demand.
    • Demonstrated conflict resolution skills. Able to adjust communication style to handle diverse situations and remain calm in demanding and unpredictable situations.
    • · Possess a high level of interpersonal skills to handle sensitive and confidential Catholic Charities, client, donor, and employee information and situations.

    • · Able to set and maintain appropriate, healthy boundaries with clients.
    • · Display a high level of initiative, effort, and commitment towards completing assignments efficiently. Possess well-developed, decision-making skills, attention to detail with a high level of accuracy.
    • Excellent organizational and time management skills to ensure multiple tasks receive adequate and timely attention. Able to function autonomously and be proactive.
    • · Ability to research, gather data, compile information and prepare reports. and utilize community resources.
    • Ability to drive - must have valid Oklahoma driver’s license

    PHYSICAL REQUIREMENTS

    • High energy and stamina to actively engage with participants
    • Lifting up to 50 pounds (furniture, household goods, computer equipment, paper supplies, etc.)
    • Ability to respond quickly to crisis situations
    • May be required to sit or stand for long periods of time
    • May be required to travel for business events or to other company locations.

    Working conditions vary based on organizational needs and may include:
