FLSA DUTIES STATUS: Non-Exempt
REPORTS TO: Director of Family Support Services
JOB OBJECTIVE: The Key to Home Case Manager will be a team member in a two-year multi-organization project that focuses on in person, housing-based case management services. As a team member, the case manager will assist in succesfully housing and stabilizing a minimum of 40 households during the two-year funding period.

SUPERVISORY RESPONSIBILITIES:
There are no supervisory responsibilities for this position.
MINIMUM QUALIFICATIONS FOR CONSIDERATION:
· Bachelor’s degree in social work, or human services related field required; master’s degree preferred
· At least 12-month’s case management experience working with those experiencing homelessness
· Fluency in Spanish and English preferred
Knowledge, skills and abilities:
· High energy and stamina to actively engage with participants and residents.
· Lifting up to 40 pounds
· Ability to respond quickly to crisis situations.
· Ability to perform CPR and the Heimlich maneuver once trained.
WORKING CONDITIONS:
· Frequent travel by foot, car or other means appropriate to contact target population regardless of weather
· Overtime when necessary to meet project deadlines
· Must be able to adjust to the environment of the target population including making visits to homeless encampments shelters and personal living environments when needed
· May be subjected to fast-paced decision making, crisis situations
